Les patients atteints de SGC d\u00e9veloppent spontan\u00e9ment une hyperplasie intestinale, une hyperphagie et une modification du microbiote intestinal. Ces adaptations contribuent \u00e0 diminuer leur d\u00e9pendance \u00e0 la nutrition parent\u00e9rale qui est le traitement de r\u00e9f\u00e9rence, et par cons\u00e9quent \u00e0 l\u2019am\u00e9lioration de leur pronostic et qualit\u00e9 de vie. Chez ces patients, la pr\u00e9servation du c\u00f4lon joue un r\u00f4le crucial dans la r\u00e9cup\u00e9ration \u00e9nerg\u00e9tique gr\u00e2ce \u00e0 la fermentation des aliments non dig\u00e9r\u00e9s par le microbiote intestinal.<\/p>\r\n<p style=\"text-align: justify;\">Dans leur article publi\u00e9 dans Frontiers in Cellular and Infection Microbiology (3 mars 2023), Salma Fourati et Johanne Le Beyec de l'\u00e9quipe de recherche PIMS, dirig\u00e9e par M. Le Gall &amp; A. Bado, en collaboration avec des membres de l\u2019\u00e9quipe de JP Hugot, ont \u00e9valu\u00e9 la faisabilit\u00e9 et l'efficacit\u00e9 de la Transplantation de Microbiote F\u00e9cal (TMF) sur des rats SGC avec anastomose j\u00e9juno-colique.<\/p>\r\n<p style=\"text-align: justify;\">Le microbiote transplant\u00e9 \u00e9tait issu de f\u00e8ces de rats soumis \u00e0 un r\u00e9gime ob\u00e9sog\u00e8ne (favorisant un microbiote efficace pour la r\u00e9cup\u00e9ration \u00e9nerg\u00e9tique). L'analyse du microbiote apr\u00e8s la transplantation a montr\u00e9 que les bact\u00e9ries inocul\u00e9es se sont implant\u00e9es chez les rats contr\u00f4les mais pas chez les rats SGC. Ceci peut \u00eatre expliqu\u00e9 par l'environnement luminal sp\u00e9cifique dans le c\u00f4lon des sujets SGC (plus riche en oxyg\u00e8ne, pH plus faible, etc.) favorisant les bact\u00e9ries a\u00e9ro\u2010tol\u00e9rantes par rapport aux bact\u00e9ries ana\u00e9robies transplant\u00e9es. La TMF n\u2019a pas eu d\u2019impact sur le poids ou la prise alimentaire dans le groupe contr\u00f4le ni dans le groupe de rats SGC. Par contre cette \u00e9tude a permis de mettre en \u00e9vidence pour la premi\u00e8re fois dans un mod\u00e8le animal la mise en place d'une hyperphagie, comme observ\u00e9 chez les patients. En effet, les rats SGC ont doubl\u00e9 leur prise alimentaire par rapport aux rats contr\u00f4les et cette prise alimentaire est positivement corr\u00e9l\u00e9e \u00e0 l'abondance des Lactobacillaceae pr\u00e9sents dans leur microbiote, sugg\u00e9rant un r\u00f4le potentiel de cette famille bact\u00e9rienne dans l'adaptation du comportement alimentaire.<\/p>\r\n<p style=\"text-align: justify;\">Les signaux issus du microbiote semblent contribuer \u00e0 la mise en place des m\u00e9canismes d'adaptation au cours du SGC dont l'hyperphagie. Une th\u00e9rapeutique bas\u00e9e sur une transplantation de microbiote pourrait donc \u00eatre int\u00e9ressante si ce dernier contient des souches de lactobacilles adapt\u00e9es \u00e0 l\u2019environnement particulier du SGC. L\u2019\u00e9quipe s\u2019attache maintenant \u00e0 identifier et tester les meilleures souches \u00e0 utiliser.<\/p>","paragraphe_en":"<h2 style=\"text-align: center;\">Lactobacillaceae could contribute to the hyperphagia observed in short bowel syndrome: an opening for therapeutic approaches based on the transplantation of simple or complex microbiota?<\/h2>\r\n<p style=\"text-align: justify;\">Short bowel syndrome (SBS), resulting from extensive small bowel resection, is the leading cause of chronic intestinal failure. Patients with SBS spontaneously develop intestinal hyperplasia, hyperphagia and altered gut microbiota. These adaptations reduce dependence on parenteral support, the gold standard of treatment, and therefore improve prognosis and quality of life. In these patients, colonic preservation plays a critical role in energy recovery through fermentation of undigested food by the gut microbiota.<\/p>\r\n<p style=\"text-align: justify;\">In their paper published in Frontiers in Cellular and Infection Microbiology (March 3rd 2023), Salma Fourati and Johanne Le Beyec of the PIMS research team, led by M. Le Gall &amp; A. Bado, in collaboration with members of JP Hugot's team, evaluated the feasibility and efficacy of fecal microbiota transplantation (FMT) in SBS rats with jejuno-colic anastomosis.<\/p>\r\n<p style=\"text-align: justify;\">The transplanted microbiota was obtained from the faeces of rats that had previously been fed an obesogenic diet, which favours a microbiota that is efficient at energy harvesting. Analysis of the microbiota after transplantation showed that the inoculated bacteria were successfully implanted in control rats, but not in SBS rats. This may be explained by the specific luminal environment in SBS colons (higher oxygen, lower pH, etc.) favouring aerotolerant bacteria over transplanted anaerobic bacteria.<\/p>\r\n<p style=\"text-align: justify;\">FMT had no effect on body weight or food intake in either the control or SBS rats. However, this study was the first to show the development of hyperphagia in an animal model, as observed in patients. In fact, SBS rats doubled their food intake compared to control rats 4 weeks after bowel resection. This food intake was positively correlated with the abundance of Lactobacillaceae in their microbiota, suggesting that this family of bacteria may be involved in the adaptation of feeding behaviour.<\/p>\r\n<p style=\"text-align: justify;\">This work suggests that signals from the microbiota could contribute to the establishment of adaptation mechanisms, in particular hyperphagia. A therapy based on microbiota transplantation could therefore be of interest if it contains strains of lactobacilli adapted to the specific environment of SBS.
