Nous avons \u00e9mis l\u2019hypoth\u00e8se que la composition et le r\u00f4le physiopathologique du gras intralobulaire (ILF) et du gras extralobulaire (ELF) peuvent \u00eatre diff\u00e9rents en fonction de la pr\u00e9sence et du stade des l\u00e9sions pr\u00e9canc\u00e9reuses pancr\u00e9atiques (ADM, PanIN). Nous avons \u00e9galement souhait\u00e9 explorer le r\u00f4le jou\u00e9 par l\u2019infiltration adipeuse dans le processus d\u2019oncogen\u00e8se pancr\u00e9atique. <br \/>\r\nPour cela, nous avons dans un premier temps explor\u00e9 les profils lipidomiques de l\u2019ILF et de l\u2019ELF pancr\u00e9atiques par MALDI-MSI et cherch\u00e9 des pics lipidiques sp\u00e9cifiques li\u00e9s au BMI et aux PanIN chez des patients atteints de tumeurs pancr\u00e9atiques malignes. Les r\u00e9sultats obtenus nous ont conduit \u00e0 r\u00e9aliser une \u00e9tude comparative entre patients ob\u00e8ses et non-ob\u00e8ses (n\u2019ayant pas de tumeurs malignes) afin (1) d\u2019\u00e9tudier les profils transcriptomiques du tissu acinaire proche de l'ILF (Ac\/ILF+) par rapport au tissu acinaire sans infiltration de l'ILF (Ac\/ILF-), (2) d\u2019\u00e9valuer le profil transcriptomique de l'ELF de patients ob\u00e8ses par rapport aux patients non ob\u00e8ses, et (3) de caract\u00e9riser les l\u00e9sions pr\u00e9canc\u00e9reuses dans le contexte d'ob\u00e9sit\u00e9 par des analyses morphologiques et immunohistochimiques.<\/p>\r\n<h1 style=\"text-align: justify;\"><strong><img class=\" wp-image-73508 aligncenter\" src=\"https:\/\/cri1149.mlcom-dev.net\/wp-content\/uploads\/2024\/05\/figure-papiersonia-frendi-2024-2-300x241.png\" alt=\"\" width=\"699\" height=\"562\" \/><\/strong><\/h1>\r\n<p><strong>Keywords:<\/strong> Obesity, Intra-lobular and extra-lobular fatty pancreatic infiltration, Pancreatic precancerous lesions, Lipidomic MALDI-TOF imaging mass spectrometry, Pancreatic oncogenesis<\/p>","paragraphe_en":"<h1 style=\"text-align: justify;\"><strong>Abstract:<\/strong><\/h1>\r\n<p style=\"text-align: justify;\">Although pancreatic precancerous lesions are known to be related to obesity and fatty pancreatic infiltration, the mechanisms remain unclear. We assessed the role of fatty infiltration in the process of pancreatic oncogenesis and obesity. A combined transcriptomic, lipidomic and pathological approach was used to explore neoplastic transformations. Intralobular (ILF) and extralobular (ELF) lipidomic profiles were analyzed to search for lipids associated with pancreatic intraepithelial neoplasia (PanINs) and obesity; the effect of ILF and ELF on acinar tissue and the histopathological aspects of pancreatic parenchyma changes in obese (OB) and non-obese patients. This study showed that the lipid composition of ILF was different from that of ELF. ILF was related to obesity and ELF-specific lipids were correlated to PanINs. Acinar cells were shown to have different phenotypes depending on the presence and proximity to ILF in OB patients. Several lipid metabolic pathways, oxidative stress and inflammatory pathways were upregulated in acinar tissue during ILF infiltration in OB patients. Early acinar transformations, called acinar nodules (AN) were linked to obesity but not ELF or ILF suggesting that they are the first reversible precancerous pancreatic lesions to occur in OB patients. On the other hand, the number of PanINs was higher in OB patients and was positively correlated to ILF and ELF scores as well as to fibrosis. Our study suggests that two types of fat infiltration must be distinguished, ELF and ILF.
