Le CHC est une tumeur h\u00e9t\u00e9rog\u00e8ne tant sur le plan morphologique que mol\u00e9culaire. Cette mise au point a pour objectif de traiter de cette h\u00e9t\u00e9rog\u00e9n\u00e9it\u00e9 en d\u00e9crivant les diff\u00e9rents sous-types histologiques et mol\u00e9culaires de CHC. Sur le plan morphologique, huit sous-types ont \u00e9t\u00e9 d\u00e9crits selon la classification OMS : st\u00e9atoh\u00e9patitique, macrotrab\u00e9culaire massif (MTM), \u00e0 cellules claires, chromophobe, squirrheux, fibrolamellaire, riche en lymphocytes et riche en polynucl\u00e9aires neutrophiles. Les autres CHC sont class\u00e9s en CHC sans signe de sp\u00e9cificit\u00e9. Certains sous-types sont associ\u00e9s \u00e0 un pronostic diff\u00e9rent notamment le MTM pr\u00e9sentant une survie br\u00e8ve compar\u00e9e aux autres sous-types. Sur le plan g\u00e9nomique, la plupart des CHC pr\u00e9sentent des mutations du promoteur de TERT, alors que d\u2019autres mutations sont observ\u00e9es plus tardivement dans la canc\u00e9rogen\u00e8se comme les mutations TP53 et CTNNB1. Les CHC mut\u00e9s TP53 sont associ\u00e9s \u00e0 un mauvais pronostic et au sous-type MTM. Sur le plan transcriptomique, deux classifications sont particuli\u00e8rement informatives car associ\u00e9es d\u2019une part au pronostic (classes prolif\u00e9ratives versus non prolif\u00e9ratives) et d\u2019autre part aux caract\u00e9ristiques cliniques, morphologiques et g\u00e9nomiques des tumeurs (classification G1-G6). En conclusion, l\u2019h\u00e9t\u00e9rog\u00e9n\u00e9it\u00e9 morphologique du CHC, en lien direct avec l\u2019h\u00e9t\u00e9rog\u00e9n\u00e9it\u00e9 mol\u00e9culaire, est corr\u00e9l\u00e9e au pronostic.
